please repeat your thyroid function test from a good laboratory, along with you need a 12 lead ecg, if its normal then ETT can be done, but all these things you need to visit a nearby cardiologist.
before investigations its very hard to say you have or you dont have any underlying heart disease

thanks for asking... you have Symptoms of palpitations on rest and exertional......structural heart diseases must be rule out. get a echo and also an ECG for arrhythmias... continue treatment for hypothyroid ... stope too much coffee tea and chocolate intake.
