Asking For Self, Male 27, Islamabad
I have a chest pain that moves around that I feel in center, left, right below collarbone, below rib bone and in upper abdomen for 3 months now, that also gives me a feeling of palpitations sometimes.
Dr advised me for an ecg, pls let me know if it's normal
ecg is within normal limits. however a normal resting ecg does not rule out underlying cardiac disease.
ecg seems fine. you need to have ETT cardiac stress test to be sure.

ETT was fine as well, did ETT test 1 month back will try to find and share. Thing is that the pain becomes managable when I walk or do some physical activity and feel more when I am resting

Any stress? do you believe you engage in negative overthinking and unable to manage your thoughts? is it that you feel this suddenly and it subsides after 5 to 20 mins? in short, it's important to rule out any psychological reasons leading to vitals fluctuations and sweating

I think I have a lot of negative overthinking, I am unable to manage my thoughts like If I had a low pain I will start searching and will try to find what this symptom is for and then whatever I do my mind will still be thinking about what's happening with me why is it happening. It happened a few years back with the same symptoms at that time as well. Every test was normal later after a few months when I somehow managed to completely stop thinking about symptoms everything got back to normal

the pain does not increase constantly but remains for a long time and every episode of pain is in different areas. For sweating I am not sure if I am excessively sweating as where I live the temp is 48+

ECG looks fine except for some changes related to right side of the heart. It is important to know about any previous medical condition that you suffer from i.e Diabetes, Hight BP or Cholesterol issues? as well as if you are a smoker or any asthma history. As such ECG does not show any signs of ischemic heart disease, in layman words no acute signs of Heart Problem.

Also if you have on and off palpitations then it should be investigated. My expertise being in Arrhythmias, can say that palpitations are the most neglected symptom in Pakistan what needs to be throughly investigated. Anyways you are welcome to reach out if you need any further consultation. Be blessed.

Thanks a lot for detailed info, I need to know what can be further analyzed for palpitations as I reside in KSA and as per my session with doctor, he said as I used to drink coffee empty stomach and I was using nicotine pouches they might be the reason that started palpitations and with time I got anxious I got used to it. I have my ECG done 4 times, only there was an elevated heart rate twice that also as per the doctor was because I was anxious. Other tests appear normal as well. Same as when bp was checked it was 150/90 initially and he told me to relax and breath deeply and checked it was 127/78 after that he took like 4 5 readings within few hours all in normal range.

As such withdrawal of caffeinated drinks and nicotine is more related to palpitations. Your palpitations need further investigation with a 24 hours Holter Monitor. Your BP seems more like White Coat but best is to do a 24 hours ambulatory monitoring. Clinic based BP check is not reliable at all. These are very basic investigations we do here in Ireland as part of risk factors assessment. Check with your nearest hospital if they have the facility of above ones.

I have been advised to limit caffeine / nicotine and use metoprolol and if palpitations persist then will go for holter monitor.
