Asalam o alikum i want to ask that how can we treat a patient with heart problem at home he is suffering from swelling feets and legs and difficulty in breathing at night and cough. He has develop edema and his heart is either beating so fast or so slow.
Can you post the reports and ECG
and short history of whatever happened in the past
please record a 12 lead ecg. echocardiography to rule of heart function.
patient is most likely has edema due to cardiac failure.
update his medications he is taking.
wsalam. this patient is probably suffering from symptoms of acute decompensation of heart failure given his previous cardiac history. he needs to be taken to some tertiary care hospital emergency and would most likely need an admission for workup management and stabilization.

WAS. As your patient is known case of IHD with may be reduced EF and currently he is suffering from pulmonary oedema and he needs diuresis which is better to be done in hospital under close observation but if it is not possible to go to hospital due to covid 19 you can give him tab lasix 40mg atleast twice depending upon BP .but try to visit any nearby hospital.
Get the diet plan for heart disease.
It's seems she has congestive heart Failure, needs proper assessment and management.
