dear u have low Ejection fraction, as well as moderate Mitral Regurgitation..
u need to do Holter monitoring, HUTT as well EEG for further workup..
u ll also need diuretic medications as well as beta blocker
Holter monitoring should be done to look for symptom correlation. EP consult for ICD implantation

The scan of the heart is suggesting significant Weakness of your main left lower heart pump.
It also suggest that the cause is unlikely a heart attack. Hence, on the balance of probabilities and limited history provided, the likely damage and weakness of your left lower main pump is caused by the leaking mitral valve which is the left upper valve inside the heart (alternatively the mitral valve leakiess could be due to tired heart).
Syncope is unusual but require urgent attention
Investigations:
ECG to rule out LBBB
Cardiac MR scan for LV assessment and viability
TOE for MR assessment
are you hypertensive?
CT coronary to rule out abnormal arteries of the heart
You will need 7 or 14 days heart monitor
Treatment:
consult a Cardiologist urgently
i recommend to consider following after consulting with expert
bisoprolol 2.5mg OD
Ramipril 1.25mg BD
spironolactone OD
If not for valve surgery, then you need ICD or CRTD devices for heart
You have to be careful with level and intensity of exertion
