On my opinion surgery is temporary relief after 6 months again need physiotherapy and rehab session
if conservative measures has been verily exhausted and still no relief then should think surgery
After viewing xray less chances of improvement with conservative treatment, better go for surgery.

Will need surgery. You can try injections with lubrication if totally unwilling for surgery. they are expensive though and no guarantee that will help.
