it's called Baby Bottle caries or Nursing Caries and is the main cause of tooth loss in early childhood.
The sugar and milk itself has caries producing potential and that when taken in bottle would promote a specific pattern where caries would grow.
you must slow down the bottle feeding to begin with and then stop it. don't give her bottle while sleeping, don't add sugar in milk. replace milk with plain water once or twice a day don't even give fruit juices to her.
your child need intense behaviour modification and councelung on brushing .
it will help her in longer run.

although the history that you gave is not complete and needs more explanation but, according to the detail that you gave I don't find anything abnormal or unusual. Still get her checked with her dentist

it is better that you visit a dentist for proper examination. and secondly, avoid feeder at night while sleeping.

it is due to sugar in her milk thats attracting bacteria to feast upon and feeder also plays a role in early loss of teeth therefore i would recommend you to give her milk in a cup and ask her to brush right after it and also visit me for treatment of her teeth

which tooth she has lost?? that might be her milk tooth that would be replaced with permanent one .and that is even normal and healthier to loose milk tooth at 5 years.. and regarding feeder plz try to move her to Glass since using feeder after 2 years is not recommended at all as that may lead to skeletal problems..now she is big and will understand things ..get her on cup..rest you can Contact and visit up my clinic to make things more clear...

If it's her front tooth, probably it's nursing bottle caries. switch her to sippy cup. don't let her sleep while drinking milk from bottle. brush afterwards. Kids start shedding their milk teeth around age 6. eventually it will be replaced by a permanent tooth but still Visit ur nearest dentist for proper evaluation.
