Sir your wife has multiple issues which require care but this tooth need immediate treatment because its infected and soon will break to the verge of being non-salvageable.
When you will visit any clinic an xray would be done to confirm diagnosis and give you treatment plan.. An rct is mostly done when the infection(bacteria) hasn't involved (eaten) roots.. So if the damage is beyond repair extraction would be advised
After rct your wide would be needing a cap (crown) as well to increase the longevity of this tooth so that would be an added cost
And after Extraction she'd be needing replacement..
She also needs improvement of Oral hygiene because that is the main reason of this infection spread..
