white spots have different causes. it can be due to high fluoride intake, if its preswnt since childhood it can be due to enamel hypoplasia, sugary and acid food can also cause white spots

increased fluoride content sometimes cause these whitish spots.
and be careful with apple cider vinegar its good that you are drinking it with straw since it causes acidic enamel erosion. 1 tbsp of ACV with 1 full glass of water makes it dilute.

avoid Taking apple cider vinegar for a wk and drink more water. it can be bcz of excessive mineral intake bcz of salty water which u might have used in early age or may be bcz of developmental defects.
it's not bcz of Vit D deficiency.
the hair loss u r having is bcz of PCOS.
