Clinical examination is must to reach final approach, although xrays suggests surgical extraction.
It is grossly carious and root is curved so it could be surgical. Clinical examination is also important.
Hey for a correct procedure choice it is important to examine clinically too. The x ray is also not that clear but from this picture it looks like partially in bone. It will require surgical extraction. Let the dentist who is performing the procedure decide what's best.

This cant be decided just on the basis of xray , clinical examination is required. Depending on that it can either be simple or surgical case.
