You have two skin disorders of the scalp - Androgenic Alopecia and fungal/yeast infection of the scalp and hair roots.

These are the reason for dandruff, itching and hair fall. First you need complete treatment of your fungal infection which may take more than 6 months. After your infection had completely gone, only then treatment of Androgenic Alopecia can be started.
