extreme weight loss in last 6 months. Urine control nahi ho raha last week se .. weakness boht ziada hai pls advise on above reports and history
Better to shift on insulin and regularly follow up by the diabetologist
Keeping in view above tests the insulin is the best option for diabetes management. Secondly diet should be advised in relation to kidney disease.

Consult a diabetologist, do eGFR test and go to a doctor.
Good glycemic control is primary requirement and as kidney function are abnormal ,needs insulin to avoid further catabolism and weight loss ,also nephrologist opinion with serum calcium phosphate and Intact PTH and try to reverse creatinine to normal if cause is possible like correction of anemia, hypertension, diabetes , cholesterol, electrolytes and dietry counseling
