Talk to Endocrinologist on Endocrinologist / Diabetologist Advice On Uncontrolled Sugar

My mother aged 70 years having diabetes more than 26 years now along with Blood Pressure. Recently, her sugar levels aren't getting in control. Her sugar levels at Fasting around 140mg/dL to 190mg/dL and in Random after 4 hours of meal comes to around 250mg/dL to 380mg/dL. Her recent HBA1C was 9.3%. She is also a high BP patient. Apart from BP & cholestrol medicines, she is using for Sugar control

Getryl 2mg once before breakfast and once before Dinner. Sitaglu 50mg (1 + 1) Dapa 5mg

Her kidneys are weak (but not on dialysis yet), and under treatment of Nephrologist for kidneys, BP and sugar. Recently she had some pains in left side of abdomen for which ultrasound and CT scans were done but everything seems normal. Doctor then stopped Metformin which he thinks not good for kidneys and can have a suspect pain for abdomen. For substitute of Metformin 500mg (1 + 1), he adviced for Dapa 5mg. But nothing has improved much and her sugar levels are not getting in normal Range. I really need
