Talk to Endourologist on Premature Ejaculation

I am 33 years old man, and married since 2008. I am also a father of 3 kids. I am continuously facing premature ejaculation problem. Before the intercourse, I attain almost full and hard erection, but ejaculated within seconds during intercourse. For the second erection, I need to wait for a long interval and also the erection is not that hard. I am now disappointed from the performance. My few recent reports are attached. I need some suggestion.
