Asking for Wife, Female, 39 years old, Karachi
I have been diagnosed with Nasal Polyps and issues in Sinuses. Both of my nose are blocked and having ear pain. There is runny nose and mucus. I am using Rhinocot spray 3 times a day. It gives releif for some time but the issues remains. Can this be fixed without surgery as I have a 1.5 year old kid to look after. If the way is surgery then what surgery is recommended?

From the ct scan it appears that you need endoscopic sinus surgery which can be done as a day case
It’s unlikely to be cured with medical treatment
However surgery is not urgent and can be done after few weeks
My suggestion is to begin Naomet nasal spray 2 puff twice a day for minimum 4 weeks and then come and see me with CT scan films

The generic name is Mometazone
Abroad it comes with the name of Nasonex in UK and USA
You can use either of them
Endoscopic sinus surgery would help you. However it can wait and u can start maximal medical therapy. This includes topical Nasal spray like mometasone, monteleukast oral tablets and an antihistamine. Also avoid triggers like dust, perfumes, etc.
Surgery is usually safe and can be done in a few hospitals in karachi. I would like to start you on oral steroids about a week before surgery.
Reaaly feel concernd abt ur issue according to ct report available u hav antrochoanal polyp which has no medical remedy only having surgical cure through endoscopy

Aoa. Your appears to have a specific type of polyp x called antro choanal polyp. Treatment is mainly done by FESS ( endoscopic sinus surgery)
After surgery recovery is fast in this case. Operation can be done as day care procedure.
Don't think to much and get it operated you will be relieved. Visit ent spec with CT scan PNS for complete evaluation
To my opinion you must take medicines for at least 4-5 weeks
Cap azomax 250mg, 1 hour before meals for 1 month
Tab atoxan twice for 1 month
Tab fload 10mg at night for 1 month
Tab prednisone 2 twice for 1 week
Then tab prednisone, 2 tab morning, 1 evening for 2nd week
Then tab prednisone 1 twice for 3rd week
Then 1 daily for 4th week
We may need CT Scan films and patient for evaluationProbably pt needs surgery

Thank you very much Shakil Sahab. I live abroad and am planning to visit Pakistan in December. I will book an appointment online to see you as I don't have medical insurance here. I am using Rhinocort 3 times a day at the moment should I replace it with Naomet which is sold as Dymista here. Also, what will be the care after the surgery and for how long?
