Talk to Gastroenterologist on Cirrhosis + Hep. C

Asking for Mother, Female, 57 years old, Lahore

Dear doctors,

My mother is undergoing her 4th round of treatment of Hep. C in the period of last 10 years. Currently she is using Viktana + Ribavirin and currently undetectable HCV-RNA after two months of medication, however, the treatment will go on for a complete six months.

She also had recently gone through endoscopy and got bands for varices. She has cirrhosis with the following reports: INR: 1.2 PT: 15 (Control: 12) Platelets: 97 Urea: 20 Creatinine: 0.9 ALT: 55 AST: 54 SWE Elasticity/Stiffness: 17.5kpa

Given the information above, I have the following queries:

1. What should be the recommended course of action? 2. Which stage cirrhosis is she in, is it early or late? 3. Are there chances of liver getting healed itself if the HCV treatment goes well? 4. Should arrangements of liver transplant be made and from where, and approximate cost?

Dear memeber go for the fresh ultrasound abdomen from a good setup
Continue with the same medication for hep c and repeat pcr after 3 Months of treatment
Follow up for symptomatic symptoms
Yes if u can afford go for the transplant option
U can not cure but halt the process of cirrhosis

The results of the ultrasound are as follows:
Enlarged spleen = 15.5x6cm
Portal vein diameter = 13mm
Liver texture is coarse
Rest is okay.

dear ahmed. your mother has compensated cirrhosis. treatment options which is available in pakistan should be continued for 6 months.
at this stage she does not required liver transplantation. after successful eradication of hcv we have to monitor for any complications by doing lab tests and ultrasound.
liver transplantation fascilities are available in pakistan.
