I am really sorry that you have to go through all this. this must be really disturbing for you. however, your hb is significantly on the lower side and it could be due to various different reasons. I do not want to create unnecessary anxiety by listing serious causes for this problem but common ones are
1- iron deficiency
2-b12 and folate deficiency
3-blood loss most commonly from GI tract
4-hypothyroidism
5-kidney problems
6-thalasaemia

please see your physician internal medicine consultant immediately for further assessment and management or do following blood tests
fbc, u&e, iron/ferritin levels, b12 and folate, esr, tsh, LFT's
urine for routine analysis, stool for occult blood.
