Does he have diabetes?
Well your father has loss of kidney function, known has chronic renal failure. He needs good BP control as well has diabetic control. Otherwise this renal failure might progress.
Get a CBC (blood test) done as well.
And visit a medical specialist in person so he'll examine your father carefully and correlate these lab reports with his condition.

High blood pressure kidney ki vessels ko demage kr skta hai jiski wja say serum creatinine increase ho skta hai. Best advise ye hai k blood pressure ko control kiya jaaye or paani ka istamal zyada kiya jaaye. Iske sath sath daily exercise ki jaaye or ghee, chiknai, or fried cheezo ka istamal kam kiya jaaye. Ye sb follow krne k 2 week baad dobara RFT report krwai jaaye.

I am so sorry to hear that your father is suffering from stage 4 CKD.
So sorry to inform you that Stage 4 CKD is not recoverable. The final treatment is only kidney transplant.
