I had a lump/lipoma in my back on upper side of buttocks 1 year ago which was very small in size without any abscess in it and there was no pain. Since i have it in other body parts also so, i took it as a genetical issue. But 2 months ago i had abscess in it and there was drain out almost 1 inch far from the lump in sacral. I consulted a doctor here in Saudi Arabia and diagnosed for Pilonidle Sinus. He recommended me to go for surgery which i am afraid of. Kindly advise me if this is really Pilonidle Sinus or not. If so, is there any other treatment rather than going for surgery?

Surgery is required if its getting infected repeatedly or its of larger size.. if its causing no problem like no pain no abscess then there is no need to get operated.

Its not big in size but when i am sitting for long time, there is swelling. Abscess usually after every 1 to 2 days i have while i get up in morning.

if it is pilonidal sinus then the only option is surgery, antibiotics can be given for Patient satisfaction but remember pus always needs to be drained and antibiotics dont go into the puss. Sinus usually have a tract so if tract is not removed then again puss will form in future.

Yes i can share the photo personally. I am a procurement officer and i am leaving my chair hardly for 10 to 15 mins in 8 hours. I have not started any kind of medication yet.

It could be a perianal abscess or may be a pilonidal sinus, as there is pus discharge so it must be drained, treatment of choice is drainage followed by antibiotics, if a pilonidal sinus is doagnosed on examination than it must be excised to avoid any further complication.if the pus is foul smelling than anaerobic cover should also be given, both these conditions initially require minor surgical intervention.

The region you mentioned is most common site for pilonidal sinus, but your question that is it really pilonidal sinus or not? can only be answered by physical examination.

The history is incomplete. Also, for the diagnosis a look at your problem is required. But if it is pilonidal sinus then excision by surgery is the only option. To prevent recurrence, you should shave the area off all hair and maintain a good hygiene.
