Asking for Brother, Male, 30 years old, Sargodha

Aoa, my brother took anti-depressants (mirtazapine and sertraline) for two months. He suddenly stopped taking those pills suddenly. It's been 2 months he left those tablets but ever since he left them he couldn’t sleep properly. He also gets panic attacks maybe. He has stopped talking about anything other than his fears about his health that he might have caught a contagious disease. We did almost his all medical tests and all were fine but he still panics about his health. His health is dropping and he remains extremely anxious all the time. Plz suggest something and what has actually happened,we are clueless

First thing, reassure the patient that nothing is wrong with him. It's his mind bothering him by making him afraid of the things he fears about himself. Second part is the treatment. Typically SSRI antidepressants are given in panic attacks, however these have to be coupled with some anti anxiety medications that you can only get on prescription. These are given along with antidepressants for the first two weeks to control anxiety. Antidepressants take effect in two weeks time, so we discontinue anxiolytics after two weeks and continue with antidepressants for at least 3 to 6 months, sometimes up to a year. Please take note that any treatment for panic attacks is NOT effective if medicines are taken any less than three months. And even after completion of the treatment, the patient may still get occasional panic attacks once in a while. So I'd suggest that you have him treated completely and reassure him about his health.

At the same time, avoid foods that contain caffeine like coffee, tea, colas and carbonated drinks. You will need to have the anxiolytics on prescription and complete the course of antidepressants. Also, only one antidepressant is needed. Mixing two or more drugs usually results in more anxiety and complicates treatment.

What your brother has is known as Hypochondriasis, which usually leads to panic attacks and is very common in the third and fourth decade of life.

Sudden withdrawal of antidepressants is the reason for his symptoms. He must see a psychiatrist so that he may be advised anti depressants again and gradual tapering off of anti depressants will improve his symptoms
