Asking for Mother, Female, 45 years old, Karachi
This post is a bit long.
so there was a splinter stuck in my mother's finger. To pull it out, she used needle (as all desi Amma do), well needle was severely contaminated that it infected her finger entirely. She was in pain, extreme pain, cried all night. It was terrible. So I took her to ER. They did X-ray. It was clear, the splinter was out but finger remain infected.
Dr advised to admit her for the minor surgery (to remove puss). She was on pain killers.
Surgery happened. They removed the puss but the there was no blood circulation in her tip of the finger.
They put her on antibiotics and consistent daily dressing.
Antibiotics is: Velocef 500, Also, Novidat, neburol forte, voren.
Now it's been a week since her surgery and dressings and medications. Her tip of the finger where they pull out the puss is turning black.
I want an opinion. Why is it happening? Is it even healing?
Medical history: She is not dietetic. No such history of illness.
get an ultrasound doppler of this hand,is she diabetic?
Get new Xray of hand, Duplex ultrasound of hand and finger.Seems distal digital phalanx is not well. Need examination for debridement/distal phalnx amputation decision.
Looking at the gangrene, she should have an amputation.
she needs an X ray & dupplex scan for the finger and hand...also physical examination is needed to decided whether to remove the dead part(black) only or to amputate the distal finger
visit a consultant surgeon nearby to properly examine if needed do x-ray for
bone involvement and also check about RBS then can plan weather only debridement is needed or extend for distal tip amputation.
unfortunately no. the tip is not healing, please get a doppler scan to see blood flow limits and its better to ask a surgeon/orthopedic to partially amputate the necrotic part instead of leaving it like this and allowing further extension. also take care of diabetes, hypertension hyper chilestrolemia etc. if any
it is dry gangrene..repeat xray of involved finger AP and lateral view.. she needs amputation( high chances). u should pay physical visit to a general/ plastic surgeon
This is gangrene .. digital amputation means kuch part finger ka utarna parey ga. no other option. reason is infection which lead to severve condition like gangrene. if may progress if we dont do surgery
non healing wound leaded to dry gangrene
will need amputation of gangrenous part

Dear this is dry gangrene secondary to loss of blood supply to finger possibly during drainage of pus.best option would be to be reviewed by vascular surgeon who will assess the demarcation and it will need amputation or removal of finger.Dr Jamal Yousuf is a vascular surgeon in Karachi at liaquat national hospital .please do visit him and he will do needful
arterial doppler should be done, amputation of gangrenous part will be done.
looks like dry gangrene. have to do some xrays n physical examination of hand for further plan of action which may involve tough decision to partially amputate finger
yes looks like gangrene , the blood supply to the distal part of has been compromised and tissue is dead of its not spreading then she just need care and ot well separate on its own

I guess, it's not spreading. I don't want the amputation of the whole finger. can we save the finger?

AOA. On the basis of your history and visual features of finger it seems that distal part of finger is not viable (gangrene) and need to be removed (amputation)otherwise it will spread out to remaining finger and hand.
