Asking For Self, Female, 38 Years old, USA
1: I am Suffering from HS from ten plus years . I flare up around my monthly cycle and the wounds don’t heal up uptil next month as I am diabetic too .i had multiple surgeries in my both armpits , both groins , under tummy and in between butt cheeks . Only on right armpit is HS Free uptill now . 2: my question is that is there any HS specialized doctor/surgeon in Islamabad or Peshawar ? Coz I am coming to Pakistan for its treatment I don’t know where to start . 3: my next question is that Can uterus Removel ( coz my HS is hormonal ) , tummy tuck ( coz my under tummy is rotten due to open sores) and gastric bypass surgery can be done in one sitting ? Co I cannot stay there more then 6 months and I have to do all these surgeries . 3: I have been through all kinds antibiotic course , spirolacrone etc and nothing worked

Good afternoon. answering your query regarding HS surgeon, plastic surgeon should go for Excisions and Skin grafting of your affected area of armpit and primary closure without grafting at belly.
you will have to discuss option with your surgeon. but Tummy tuck should not be done ideally along with excision of unclean area of HS.
it's hyderadnitits supperativa it's advance infection treat it by antibiotics and cleaning if scarring and advance cases you may need skin grafting after complete excision .

dear you may try hormonal therapy like estrogen and progesterone combines ,so you get your hormones suppressed.plus anti inflammatory like Nsaids and spirnolactone , zinc and vitamin E combined
continue it for at least 3to 6 months
other wise excision and grafting is ultimate solution..

What do you mean by hormonal therapy ? What exact medicines should I ask from my doctor so I can buy the medicines from here . Other then that I have nsaids , spirnolactone and zinc at home . I’ll buy vitamin E and kindly tell me the dosage ? I have two months before coming to Pakistan . So if this regime works then I’ll be spared from more surgeries

hystrectomy and tummy tuck can be done at the same time, if the surgeon is daring enough, but sleeve gastrectomy is a seperate procedure n should be taken seperatly. i advise you to get tummy tuc, n vaginal hystrectomy.n think about sleeve gastrectomy later.
well, for HS, treatment options have been used already, any experienced surgeon can repeat it as necessary.
I think u should not go for syrgery again. Initially some effort should be done to contol infection. Then surgery can be done after one month.

What do you suggest ? I am 24/7 in pain and tried different kinds of pain killers . I am really really exhausted of this disease . Cannot walk , sit not sleep

First of all, really sorry to hear about your condition.
Regarding HS specific expert, there is none as such but I would recommend to get an expert opinion from Dr Mamoon Rashid Program Director Plastic surgery Shifa International hospital.
I would recommend not to go for 3-in-1, but rather 2-in-1 may be feasible i.e., hysterectomy and tummy tuck as a joint venture of ObGYN and Plastic surgeon.

You know that Doctor Manoob Rasheed had done HS surgeries ? And why is this disease not acknowledged by anyone ? I mean when I was in Pakistan five years back , I went to so many doctors / dermatologists / gynecologists and nobody could recognize this disease.

Yes I do but I won't call myself an expert in it. I would still suggest go for plastic surgeon, and mamoon rashid is your best bet.

First of all u need to have good diabetic control. you need exision of HS may be in two steps followed by local flap rotation or skin graft. Hystrectomy can help after healing of first operation. remaining surgeries should be done later if at all needed

sorry there is no such specialist. but your uterus and tummy tuck can be done in one sitting, better is that you reduce wait, u can get Laproscopic wt reduction surgery followed by tummy reduction
since you have been going through this for quite along time , you would have definitely tried antibiotics, so excision and grafting but be the option.
