Talk to Gynecologist on Intercourse

Posting on behalf of my cousin
My cousin is 31 years old. Whenever she has intercourse, she faces the issue of massive bleeding with extreme pain. It is from the beginning & happens everytime. She could not even enjoy it. She & her husband are very upset. They come from a backward area, that is why they feel hesitant to visit doctor.
Kindly tell me the possible reason or issue behind it. What should they do?
