Low haemoglobin also called anaemia.
Grade 1 anaemia: haemoglobin less than normal for age and gender up to 9.9 g/dL. This is mild anaemia. No Investigation needed. Just improve diet, give haematinics. Rule out renal disease, Liver disease, Thyroid disease, thalassemia minor Or any drug induced cause.
Grade 2 anaemia: haemoglobin range from 8.9 to 9.8. This is moderate anaemia. Investigate cause which might be combined nutritional deficiency Or other above-mentioned causes. No blood transfusion needed at this level.
Grade 3 anaemia: haemoglobin range from 7.9 to 8.8. This is moderately severe anaemia. Causes maybe Hemolysis, acute or chronic blood loss, along with above-mentioned causes. No need of transfusion at this level Unless a surgical emergency is there. Treat the cause.
Grade 4 anaemia: Haemoglobin range from 6.9 to 7.8. This is severe anaemia. However, no transfusion needed even at this level Unless there is some surgical emergency. Causes include thalassemia intermedia or major, G6PD deficiency, Hereditary Spherocytosis and above mentioned ones.
Grade 5 anaemia: Haemoglobin level 5.9 to 6.8. This is very Severe anaemia. Blood transfusion may be needed according to the Cause. If there is production disorder, Transfuse is the patient. Transfuse in Thalassemia major. No need of transfusion in mere combined nutritional deficiency or renal disease. Also assess cardiovascular system at this level.
Critically low haemoglobin or grade 6 anaemia: haemoglobin level less than 5 g/dL. Transfusion is needed regardless of the cause. Consider PRCA, thalassaemia major, aplastic anaemia.
The work up for anaemia includes CBC with red-cell indices, platelets, reticulocytes and peripheral blood smear report, LDH, hematuria work up, and melena/hemetemesis/hemoptysis work up, bone marrow biopsy report, Ferritin, vitB12 and RBC folate levels, RFTs, LFTs.
