Talk to Internal Medicine on Chronic Illness

Hi Marham doctor's team My question is. My parents are diabetic prolong on insulin. They have multiple problems both of them. Like chronic illness, insomnia, depression, osteoporosis, stomach disorder, iron and vitamind3 deficiency. High blood pressure, and many more disease. Plz suggest doctor and department. Which department treat in one window and manage all problems. I'm very confused. Some people say it's BLOOD DISEASE issue and some says it's general medicine. What I do. Is there any other field to solve these problems and a good doctor also very good practice and experience. Plz guide me what I do. Thanks

OK here's how it works. People who specialize in internal medicine are called physicians. General Physicians or GPs for short are doctors with simple MBBS or an advanced degree like MCPS in family medicine or MRCGP. In contrast doctors who have done FCPS, MRCP or Diplomate American Board in Internal Medicine are called Interns in United States but simply Consultant Physicians in Pakistan. Doctors who either specialize in a particular area like Heart or Brain or Lungs may do so after becoming interns or directly specialize in the field or just get experience in one area. These are then called cardiologist, pulmonologist, endocrinologist etc. Pretty confusing huh.

So what you need to do for your parents, if you think GPs (simple MBBS) people cannot help them anymore is to go see a Consultant Physician - an Intern: Someone with a Post Graduation in Medicine. An intern would be able to relate all problems going on and would be able to balance out the effects of treating one over the other. If they require help in a particular area, they might involve a cardiologist for the heart, an gastroentologist for liver, stomach or intestines, a Nephrologisdt for Kidneys, an Eye Surgeon for Eyes etc. but the Intern would remain your primary physician balancing out all advice from different specialties and may later ask you to stick with one sub specialty if your parent's major problem is with that specialty.
So choose a Consultant Physician. Hope this was not too confusing and did actually help.
