Assalam o alaikum,
There is no history attached BUT with the information provided it is obvious that the patient has poor functioning kidney due to stone. The obstruction seems to be old as suggested by thinning of the cortex. Overall kidney function is still normal as the other kidney has enlarged (due to a process called compensatory hypertrophy. This is the phenomenon commonly observed after kidney donation when only one kidney remains). Now there are two issues.
1. What to do with this kidney stone?
2. How to preserve the kidney function of the good functioning kidney?
A urologist may answer the first question better, as it may require surgical removal, particularly if there is clinical evidence of infection.
For the second question, she should avoid certain pain medications in the longterm (the group called NSAIDs, such as brufen, voltaren, etc). Always consult a doctor or pharmacist for safety of pain medications, particularly when required long term. She should always keep well hydrated to ensure that her urine is not concentrated to avoid further stone formation. Shebmay require other tests to know if she has a tendency for recurrent stone formation.

Rt kidney Kaam nahi kar rhi aur us ki waja stone hai . Iski wapsi nahi ho gi . Baqi kidney tests ok hain . Urine test mei infection a rhi hai. Clear mashwara ye hai k kisi achay urologist ko dikhaein aur May b wo right kidney ko remove krne ka mashwara bhi de sktey hain aur agar aisa ho to foran decide kr k manage krwaein .

This should be detailed history with the objective of identifying
1. Is there active infection?
2. Is there a secondary cause of kidney stone formation (other than the fact thatvit is a malrotated kidney)?
3. Is there history of passage of stones in the past or a family history of stone disease?
4. Is there constipation? Or psychosis? Or anything suggesting hypercalcemia?
And many more.....
It would be best to see a urologist face-to-face as everything can not be sorted out online.
