4mm kidney stone generally doesn’t cause significant blockage and is easily expelled by drinking enough water and certain medications.
I would recommend to get a CT KUB without contrast to see the exact number of kidney stones and their sizes. Further treatment can be decided depending upon the CT report. Thanks

Pl send CT KUB images with renal protocol, after which can be suggested whether conservative treatment or any intervention is required.
4 mm stones are less likely to cause such massive hydronephrosis.
These can be managed with meds most probabily if these are really of that much size
