Please write down the whole story in your own words. What exactly are her symptoms? How long has she been ill? What treatment have you been advised? What investigations have been performed including blood tests, X-rays, scans etc? Has she been seen by a neurologist before? If so what were you told? Just saying brain problem doesn’t mean anything. If you can’t write the history yourself then get someone to help you
