asslamo allycum, for back pain we needs MRI scan of lumbosacral spine . it looks like that your father is also suffering from knee pain and hip pain as well as was shown in Xrays . He needs examination by orthopaedic surgeon. I am in UK , you can find online appointment with me or wait until end of April during my visit to Pakistan. If he is severe pain then find local surgeon. Disc desiccation changes are seen at multiple intervertebral disc levels.
modic change seen at l4 l5 vertebral l3vel.
schmoral node seen at l4-l5 level.
A suspicious looking T1w& T2W hypo intense focus seen in intra dural region at D12.
posterior disc herniation seen at l3-l4 & L5-s level having central & bilateral para-central components causing impression on spinal canal & bilateral exiting nerve roots of mild to moderate severity.
no spinal canal stenosis
no abnormal intensity signal in spinal cord
posterior elements are intact
conus lies at normal level
rest of study appears unremarkable

it is multiple level disc issues. Two options injection plus physio or surgery plus physio, depending upon symptoms and examination of the patient. kindly see any surgeon if your father can not cope with pain. or find appointment with me on line or see me in April in mujahid hospital Faisalabad.
