Chief Complaints Recent Episode (1 Month Ago): Sudden onset of palpitations, heart rate of 195 bpm, blood pressure of 187/120 mmHg. Symptoms included cold sweats, feeling faint, heat rushes, and fear of passing out. Diagnosed with a panic attack at the ER; symptoms improved after receiving IV treatment.
Ongoing Symptoms: Frequent feelings of detachment (out-of-body sensations). Cold sweats, difficulty concentrating, and episodes of bad moods. Persistent fear of having a heart attack or cardiac arrest. Generalized anxiety about heart health and mortality.
Past Medical History Hypertension: Mildly elevated blood pressure, currently managed with medication. Hyperuricemia: Elevated uric acid levels (8.60 mg/dl). Recent Echocardiogram: Mild concentric left ventricular hypertrophy, normal left ventricular systolic function (LVEF 60%), mild diastolic dysfunction, no significant valvular abnormalities.
