Asking for Sister, Female, 37 years old, Lahore
My sister is taking medication for bipolar issue. She is taking medication for 4 years. She is all OK as long as she takes medicine suddenly she stops medication but shows that she is taking them. When not on medication she doesn't sleep, eat well. She keeps going through things in cupboard,throw things away. Misbehaves with parents only shout at them and keep talking about past and the talk doesn't make sense. She was kept for treatment in a hospital for almost a month. But she has an issue of discontinuing her medicine. She got divorced because of this issue. Medicine she takes are neurolith, olanzia and revotril. Can someone help with her issue please. Half a year she is not fine. It becomes very difficult for her and the family. She has children too.

Combination of medicine shows ,you have poor follow up with psychiatrist.
Poor compliance to medicine is a common issue in such patients.

But most important is good follow up with appropriate and qualified doctor who will find the underlying problem with poor compliance.

Get her serum lithium levels done. Make other ways to ensure compliance to medication. Take consultation of a psychiatrist for this to make a proper management plan
'Not taking medicine properly' or Non-adherence to treatment is because of multiple factors both pharmacological and psychological, so both of these factors need to be addressed.
Also adequate follow up with a Psychiatrist for ongoing review of medication and any side effects of medication need to assessed on a regular basis as this can be a very important factor in non adherence to treatment.
