Asking for Brother, Male, 18 years old, Karachi
My brother was diagnosed bipolar back in 2020.after 6 months treatment from doc yahya amir he came to normal life.medication had been stopped by doctor.back in October 2022 he again became fast from then he is in medication but he remained well but not completely well for the last two days he is again fast.he is taking lamotringe 100 mg and depex 20 mg per day. Kindly guide should we change psychiatrist despite in medication how relapse comes plz

difficult case, certainly medications needs looking at , choose any experienced and qualified psychiatrist,it could be me for second opinion and medication review , I think he can be stabilized within few days to weeks but without face to face consultation definite statement cannot be made
you must consult with psychiatrist. medication must be reviewed.
In bipolar with Mania or high condition, antidepressants like depex shouldn't be given. number 2. in bipolar the treatment with mood stabilizers should never be stopped for at least 4-5 years. number 3. the follow ups are essential at least every quarter or with every season change.
if your psychiatrist not following these , change your psychiatrist.

Bipolar Disorder has a cyclical pattern, where the mood changes between Depression (low mood) and Mania (elated / high mood. Medication and therapy help to manage the illness, and allow the patient to limmhace have have adequate functionality. The treatment is usually for a long period of time with periods with few or no symptoms.
Even the slightest stress can trigger the symptoms to return.
I would suggest that you discuss your concerns with his present Psychiatrist. There may be a need to adjust the dosage, etc.

Your brother aged 18 is on good mood stabiliser but dose is low secondly start risperidone with dose adjustment or Quetiapine with dose adjustment and inshallah he will be fine .hopefully he is physically fit and fine and thyroid functions are normal .Share it with your psychiatrist see if he finds this a good guidance.All the best
Bipolar affective Disorder is episodic with illness episodes alternating between depression and mania/hypomania. Even if a patient is on medication and compliant to it, stress of any kind and any level as well as disturbance in daily routine specifically sleep-wake pattern can trigger a manic episode. A psychiatric illness is unique in many ways. The clinical picture changes over time and unfortunately the illness runs a lifelong course in many patients. It is wise to stick with your first psychiatrist since he knows the patient firsthand and knows how the illness started, specially if your patient remained stable on medication advised by him/her. If you are no longer satisfied with your primary therapist or the response is inadequate as per your expectations, you can always take a second opinion. Good luck.
