Are automatic and intrusive smells act in the same way automatic/intrusive thoughts work in OCD patients? This means as the intrusive thoughts are fake so are the intrusive thoughts? Thanks.

It's a possibility, It can be a symptom of Olfactory reference syndrome which comes under OCD
