He may have some kind of Personality Problem. there are mixed symptoms so talk to him and ask him to consult a psychologist. but I let me tell you personality issues are always observed by the society or other person not the one having it so it is bit tricky to confront or even talking to the person. aise ho sakta hai wo apne in sb cheezo ko normal samjte houn unhe in se masla he na ho. per as you mentioned he cuts his arms so that's a thing to be handled on immediate basis. better to consult a psychologist.
