granuloma is mostly due to tuberculosis.
Emphysema is due to smoking or working in fumes or smoke.
Thickening of costophrenic angles is due to fluid in old infections like tuberculosis.
But important is that,as pulmonologist, x ray chest reporting has little value.First thing is patient history,signs and symptoms and secondly x ray film not its report.

Thanks for your reply. I went to aga khan had an appointment with Dr Tauseef he guided properly that he has to stop smoking and prescribe some medicine.
Is he smoker or Ex smoker. Need spirometry test to assess lung function and then will start treatment.
