Talk to Gynecologist on Bleeding After Stopping Heavy Bleeding After Stopping Med Roxy Progesterone

A female 22 diagnosed with PCOS 2 months ago was prescribed spironolactone (for hair loss) and medroxy progesterone. She hadn't had her period for 6 months prior to the medication. She took the medication for 1 month and then stopped. 20 days later she started having excessive bleeding. Which hasn't stopped now for a week. Her BP dropped to 70/50 yesterday. She has been experiencing a lot of dizziness.

She has been taking 2-3 Litres of water to compensate for fluid loss. We just gave her oral tranexamic acid 1000 mg an hour ago. Her BP was 120/80 The bleeding hasn't stopped yet. We plan to give an injection in the evening in case the bleeding doesn't stop.

What are you recommendations for short term and long term management
