Talk to Urologist on Urology Issue

I am having urine leakage. Moreover I can't feel any sensation of touch on scrotum, penis and some area above it. Pinching is felt. Scrotum and penis seems darker and inner thighs also appears dark near scrotum. Tubes connecting testicles feels like intermingled noodles but when I lay down the testicles sag and the tubes seems ok. There is dull kharish on whole area. I feel irritation when I lay down whereas it feels ok while standing.. No sexual history. No burning during urination. When sagging the penis become a little enlarged and the scrotum skin sometimes gets akhti around testicle. Testicle sometimes feels soft other times gets hard.. There appears veins on lower side of both testicles skin. Have constipation too. The issue is from 1 year.
